Welcome to another exciting season with Loganville Youth Football Association. Please read this information carefully and keep it handy so that you can refer back—it has important dates and information to answer your some of your questions.

| REGISTRATION IS CONSIDERED COMPLETED, WHEN WE HAVE ALL OF THE FOLLOWING FROM YOU: BIRTH CERTIFICATE, COMPLETE PHYSCIAL FORM, SIGNED CODE OF CONDUCT, REGISTRATION FEE PAID AND SIZING FOR UNIFORMS. NO PLAYER WILL BE ALLOWED TO PRACTICE OR TRYOUT UNTIL ALL FORMS MENTIONED ABOVE ARE COMPLETE. |
| REGISTRATION FEE: $180.00 (this amount includes registration, 2 jerseys-black & white, $40 spirit money). We accept checks, cash or credit cards. Make all checks payable to LYFA. All returned checks will be assessed a $25.00 service charge. The spirit fee pays for different things such as: lettering of the jerseys, Gatorade, ice, cups, drinks, and/or food for the pep rallies, end of the season gifts and coaches’ gifts. This fee is to cut down on our asking you for money later. There will not be any refunds.. |

| BIRTH CERTIFICATE: We must have a certified copy of you child’s Birth Certificate. This is a copy with a Raised Seal certified by the court clerk. We will make a copy for you but must see the certified copy. If birth certificate is not available, you can get a letter from the Principal of your child's school. It MUST include statement of his age and it MUST be stamped with the OFFICAL Seal from the Principal. If your child's birth certificate/letter from the school is NOT in his file then he will NOT be able to attend evaluations or practice. |
PHYSICAL: A GFL physical form must be on file before your son can practice/evaluations. You can use your own doctor for the physical; however, the doctor must use the GFL form. We offer a Physical Day and will have a doctor here available for a sports physical. This will be on the last registration day, June 26, 2009 from 10 am –12 pm. Cost is $15 approx. This form cannot be signed by a nurse, registered nurse or nurse practitioner. Must be completed by a Doctor or Physicians Assistant.
No one will be allowed to participate in tryouts or practice without the physical and the birth certificate on file. You can obtain a physical form at www.gwinnettfootball.com. |
PHYSICAL DAY & EQUIPMENT DAY: New game jerseys will be ordered. When coming to any registration day, please bring your child for sizing of new jerseys. New equipment will be available on Physical & Equipment day only. Lee Sports will be available that day only to size for new equipment and they will have packages available that will include cleats. The following is a list of equipment that you will need to purchase for your child to play football.
