Welcome to another exciting season with Loganville Youth Football Association. Please read this information carefully, some things have changed, and keep it handy so that you can refer back—it has important dates and information to answer your some of your questions.

| REGISTRATION IS CONSIDERED COMPLETED, WHEN WE HAVE ALL OF THE FOLLOWING FROM YOU: BIRTH CERTIFICATE, COMPLETE PHYSCIAL FORM, SIGNED CODE OF CONDUCT, REGISTRATION FEE PAID AND SIZING FOR UNIFORMS. NO PLAYER WILL BE ALLOWED TO PRACTICE OR TRYOUT UNTIL ALL FORMS MENTIONED ABOVE ARE COMPLETE. |
| REGISTRATION FEE: $165.00 (this fee includes the $40 spirit fee). We accept checks, cash or credit cards. Make all checks payable to LYFA, all returned checks will be assessed a $25.00 service charge. The $40 spirit fee pays for items such as: lettering of the jerseys, Gatorade, ice, cups, drinks, food for pep rallies, end of the season gifts and coaches’ gifts. The spirit fee is maintained by the team mom and used on things that are decided by the coach and team mom. This fee is to cut down asking you for money later. There are no refunds on registration. |
| CONDUCT: Please read & sign the "Mission Statement and Code of Conduct". We hold everyone involved with LYFA to a standard of conduct. We expect parents and players to follow these basic guidelines. |
| BIRTH CERTIFICATE: We must have a certified copy of you child’s Birth Certificate. This is a copy with a Raised Seal certified by the court clerk. We will make a copy for you but must see the certified copy. If birth certificate is not available, a letter from the Principal of your child's school can be used. It MUST include statement of his age and it MUST be stamped with the OFFICAL Seal from the Principal. If your child's birth certificate/letter from the school is NOT on file with us then he will NOT be able to tryout or practice. |
PHYSICAL: A GFL physical form must be on file before your son can practice/tryout. You can use your own doctor for the physical; however, the doctor must use the GFL form. We offer a Physical & Equipment Day and will have a doctor available for sports physicals. This will be on the last registration day, June 27, 2009 from 10 am –12 pm.
No one will be allowed to participate in tryouts or practice without the physical. You can obtain a physical form at www.gwinnettfootball.com. |
EQUIPMENT: New game jerseys will be ordered. When coming to any registration day, please bring your child for sizing of new jerseys. Other new equipment will be available on Physical & Equipment day only. Lee Sports will be available that day only to size for new equipment and they will have packages available that will include cleats. You will not find the equipment you need at a cheaper price. The following is a list of equipment that you will need to purchase for your child to play football.
